Promotions from non playable leagues

Hey I have got a question about teams being promoted (or demoted) from leagues that arn't loaded or are not playable.

Mainly, how is it decided which team or teams get promoted from a league which is unplayable? For Example, in Italian Serie C2, at the end of the year, how is it decided who gets promoted from semi pro to Serie C2? I have a couple Italian semi pro teams and last season one of them (Fano) was promoted to C2 (which I'm trying desperately to keep them from being demoted).

I think it does the teams with the best reputation and stuff, stupid really :/

It's difficult choosing feeder clubs when they arn't in a league. Really it's just a guessing game. They arn't worth squat unless they are in a league so you can send your players there on loan.
