sounds like your defenders are pushed too high up or there closing down setting it set too high?

try changing the defensive line bar down afew clicks & changing the defenders closing down bar to about half way

Have a look at the supply. Who is making these t-balls, is he getting too much time on the ball?

I think this is an ME issue with zonal marking. I have 2 excellent CBs and I would say half of the time this happen to me too, but because my CBs are fast they can quickly chase down the through ball and close down the other player.
