I would've though this is a fairly simple question to answer.
Player's condition is low, sub him.

Depends really, Sometimes the ratings thing can be wrong its up to you whether to follow the ratings thing and take off the player that has the lowest and etc. I tend to just watch my games carefully and see which players are playing decent and where others aint then i make the decision on my own without the ratings helping me..
When your holding onto a lead sometimes its best to leave the players you got on than to make a sub all you nee dto do is adjust the tempo and passing style and turn on time wasting and put a 'senna' type player in between the lines midfield and defense. Because sometimes when you make a sub when leading 1-0 it can risrupt your teams rhythm and could lead to letting a last minute goal in its best to make little changes.
