best way to counter attack in the epl?

Fast strikers, playing as high as possible
 
What sort of attacking players do you have at your disposal? And also have you got any sort of playmaker?

Mate, you have some sort of creativity in Hosny or McGugan, make them your primary playmaker.

Set to counter, play a 4-1-4-1 or a 4-5-1, stop offensive moves, then get the ball to a creative player.

This is where you have quality, out wide. Lalana, Danny, Chamberlin and Albrighton are all quick pacy wingers, use their pace to get round the back of teams, and stick Dembele as a poacher and sneak goals that way.
 
Set your tempo as high as it will go.. and target man as run onto ball.. then focus passing down the wing with your fastest winger.

what roles would you suggest for the defence and midfield if i did a 4-5-1?

Id have the defence not commiting forward, especially LB and RB, and probably the decent cover/stopper idea.

Your DM should be an anchor man, and have one of the CM's as a defensive midfielder with support, so he supports both attacking and defending movements, your other CM as an advanced playmaker with either support or attack, two wingers with attack and the attacker as a poacher.

I'll be honest mate, I haven't tried it, but I went for all out attack and won 1 game in the last 12, so this is how I would set out if I had a second chance.

It looks decent mate.

What I'd reccomend is two things, one, it might work and if it does let me know, but I would implement it against stronger sides.

Two, if it doesn't work, you evalute it on more a game to game basis, and go for it more when you feel you can grab a win. I mean, youre 15th, I struggled to get higher than that because I didn't try anything different.
 
Ahhh man, it's not really the best idea in the first place to play defensively against a "weaker" side.

The counter idea is good to keep you in the league when playing stronger opposition, but play the way your players are used to playing when playing those sides you should be beating.

My players are not used to playing anyway they always do ****.
