Conceding Late

Been running into this issue with Milan in my save, i go up one or two nil and then concede whatever the margin may be within the 79-90 minute span, any suggestions or reasons why this would keep happening?

You most likely have players with low stamina. Changing formation and bringing on defensive players in this period could help.

Another issue may be that your players have low concentration levels. Focusing training individually in these areas, especially defenders, would probably help. May even turn the tables around, and you could notice you are the ones scoring in the last few seconds
