Yes, for Emmanuel Mayuka (or someone else, can't recall atm), when I was managing Soton. There are very few things you can do.. The player needs to be playing regularly for his country, in order to gain the WP (doesn't matter what level - Senior, U21, U19 etc.) If he isn't, you're best off sending him out on loan, till he starts playing for his country again. Occasionally, just applying for WP from his profile page can get you the permit, but it varies a lot.

No - it has to be in the A-team, and the team needs to have a certain world ranking.
In other cases you can get permits if the talent is very gifted/special - but this only happens to premier league/championship teams and with very talented players.

A useful way of getting WP is to get a feeder-club in ex. Belgium and loan the player out for a period of years. But this could damage the development of your player.
Try surfing the forum or google - sure you can find a guide that explains it all.
