Rules were changed by UEFA after Chelsea won the CL and knocked Tottenham out of it. This is now implemented in FM. It is now possible to have 5 teams in the CL if it happens again with a team finishing outside 4th but winning the CL.

The only way a team in 4th can't get Champions League football is if two teams of that same league outside the top 4 win the Champions League and the Europa league.

Previous season on my save had SIX epl teams qualify. Top 4 as usual, with the 4th placed team not losing their place; 5th place won the Europa league and qualified as a result and 6th place won the Champions Cup.
