I'm managing Celtic at the moment and we've just finished our first CL Group Stage match (unfortunately losing 3-2 to Arsenal) and it was during the next SPL game that I noticed that condition of most of the team was 6-12% lower than the other team's players. I haven't changed anything in the training except letting my assistant handle individual training. I've also been rotating players to ensure they get match time. I don't know what I'm doing wrong although I'm new to FM and have only been playing since Monday.

it might be worth checking the extended leave in the players tab pf your club to see if any of your players need a break to get fitness back. Also check the players match condition and fitness as well as their natural fitness and stamina stat. That should help you stop it on top of monitoring their fitness in the game and if it doesnt change try reducing the training workload giving them more time off
