No cover for players receiving treatment?

Just conceded a goal because one of my CBs was receiving treatment for an injury picked up attacking a corner. In real life one of the midfield players would drop in to this position whilst he is off the field, but here my team just left a ridiculous gaping hole in the back line which an opposing striker just walked through to score.

I was not running a 'rigid' system, so what the **** can I do to prevent this?
 
You should manually drop the midfielder back, that seems like what you should do?
 
Never seen that - I've always seen midfielders drop in to cover the space when there's a hole, particularly in the back line. In all the time playing FM I've only conceded once when a defender is receiving treatment (I know it was once because I remember how ****** angry I was).

Are you sure a midfielder didn't drop in to that area, then, not knowing how to play centre-half, launch a **** sliding tackle that left him stranded way out of position?
