I use the same formation, however I've set my fullbacks as wingbacks (they are pacey and can handle it) but if I make a sub to a slower sideback I switch them back to fullback.
Also I've set my defensive minded midfielder to be very defensive in mentality (almost full bar defend). If you look at average positions he will play just like a DMC except more aggressive in challenges (needs good "decision" stat to avoid cards) He usually doesn't get any assists or goals, but that is not his role anyway!
I've also set the AMC and two strikers to have free roles, as the lack of width can cause a bunching up near the 18 yard box without the free roles. The pacey striker usually breaks out wide, cuts in for shots, or crosses to the charging AMC, MC, or other striker.
My main problems with this formation has been with the offensive minded MC not always making good runs, and players bunching up on offense as I mentioned before. Also my primary playmaker (amc) can often get lost in a game and not get many touches (longer passing + poacher is target man set to run onto ball, doesn't help that issue)
