For example, say if you have a player and there nationality is, italian but their "other" nationality is welsh, how do you make it so there main nationality is welsh like instead of italian?

You can't unless the said player gets called up for Wales before he gets called for Italy

They gotta get capped by wales, which is a mission because out of 2 nations the player always wants to play for the higher ranked one.
