Sometimes when I advance time (by just pressing 'continue') I'll come back and find all my youth players have been moved to the senior squad. It used to be just an annoyance because I had to rethink who was in U21 and who U18, but now it's doing it right before an U18 match so I'll set my dudes as available, click 'continue' and come back to find everybody in the senior squad and the match already played with those dark grey filler players. I've reloaded a couple of times and it keeps happening.
