I was thinking as there are four variations of the 4-4-2 that I would personally use:

Flat 4-4-2:

Which of these offers the most balance in both defence and attack, particularly the last two which I favour due to DM being most of the defensive midfielders favoured position. With the last two does it make a difference/improvement if the DM is directly behind the central midfielder or deeper to either side of him?

I can't see the pictures so not sure which ones you chose...What about the 4-4-2 diamond? With wingers or CM's? Or what about a 4-4-2 with two DM's and two AM's?
 
i pllay the diamond system with a good holdeing midfeilder u cant go wrong
 
Flat 4-4-2 is the most balanced one, but really boring to watch. I prefer 4-2-2-2 one because you can set the wingers to inside forward
 
the one with 2 pushed up wingers playing as inside forwards by a long shot - having one of the CM's as some form of defensive-minded one.
