This screenshot doesn't show consistency. If he performed in fist game average rating of 9 and next two matches he's got 6.5 and allowed two goals, the average for the three matches will be 7.33. In fact, FM 2014 has a problem with ratings. They are inconsistent and sometimes aberrant. I had a left back who was rated with 6.1 and my assistant said that's because he failed to pass to his team mates. I found in this player's statistics that pass attempted was 32 and pass succeeded was 30. Besides, I saw all that game, extended game and he played very well. Those two of his passes were intercepted. My team was winning and I didn't allow any goal.

And this is just his recent form. He's been like this for 5 years. I can't remember ever seeing him below 6.5, and even those ratings are very rare.
Not sure why I should consider myself lucky. His injury proneness is set to 0, meaning it will randomize from game to game. So you can't say he's injury prone as a default. Which by the way is also the case for his consistency. So saying he's inconsistent and injury prone simply isn't true.
Ok, I give up. I have two saves and in both he wasn't play very well. Probably, there are some random variables which affect the game.

Consistency and injury proneness is set to 0 in the editor. This means that they will randomize from game to game. In one game his consistency could be 20 and his injury proneness 1, meaning he'd be out of this world, will always perform and never get injured. On the other hand, he could get a consistency of 1 and an injury proneness of 20, meaning the opposite would happen.
So since both his consistency and injury proneness are randomized from game to game, we can't say he's inconsistent or injury prone as a default. In other words, how he performs in your game (or mine, for that matter) could be completely different from someone else's game. But one thing we do know is his potential, which is -10, the highest possible.
